How Does A Spinal Cord Stimulator Trial Work?
Before implanting a final spinal cord stimulator, the person needs a trial stimulator. The trial lets the doctor know if the person will do satisfactorily with the final implant. The usual rule of thumb is if the person gets fifty percent or more pain relief with the trial, the the final SCS implant is indicated.
The SCS trial is done in as an outpatient by an AZ pain management doctor trained in such procedures. The patient receives IV sedation but cannot be knocked out fully as the physician needs input from the patient during the procedure. The patient is placed face down on a procedure table that allows x-rays to go through it (radiolucent). The procedure is performed with a fluoroscope (real time x-ray machine).
The soft tissues and skin are anesthetized fully and then a guide needle is situated through the numbed track. Through this guide, the SCS catheter trial is pushed and manipulated into the epidural area, which is the region around the spinal cord. The real time x-ray machine (fluoroscopy) assists with catheter placement, as the catheter contains metal which shows up to assist.
Once the catheter is placed in the initial position deemed acceptable, the representative from the spinal cord stimulator company being used passes a sterile power attachment into the operating field and the physician then turns it on and initial programming of the electrical stimulation is performed.
The patient now becomes an active participant in the procedure. While sedated but not asleep, the individual responds by telling where the electrical sensation is felt. The SCS catheter is moved around by the pain doctor and situated so the person feels the electrical coverage where pain is typically felt on a regular basis. This may be back pain, leg pain, or both.
Once placement obtaining adequate pain coverage is set up, the SCS catheter is sutured in place and sterile bandages are put in to place. For the next three to five days the trial is left in place as an outpatient, receiving electrical stimulation to see how it works at pain relief. The spinal cord stimulator catheter is hooked up to a programmer battery pack which attaches to the person’s waist.
On a follow up visit to the AZ Pain Doctors office, the catheter is removed in the office with a wound check. The patient then relays to the Arizona Pain Center doctor how much pain relief was achieved and the decision is made whether or not to proceed for a final spinal cord stimulator implant.
